Consent Form For My pet's Dental Procedure & Anaesthetic
Thank you for choosing our team to help you care for your precious pet, we appreciate the trust you have placed in our team and we are committed to providing the highest standards of veterinary care at all times for your pet.
Before we can proceed with your pet's dental procedure, we request that you choose the surgery below and complete the consent form in full. This will ensure that our team have a full view of your pets medical needs, you have the time to review the consent required and that you are comfortable with the proposed treatment plan.
Our vets will only remove teeth when it is necessary to do so, unfortunately, it is not always possible to be certain about how many teeth need to be extracted until we have removed the tartar and cleaned your pets teeth. Following the removal of any tartar, our team will then measure the amount of pocketing and bone loss around each tooth. Significant bone loss or teeth showing areas for concern we will recommend removal of, as these issues will cause chronic discomfort if left untreated.
Our pricing structure can be viewed below. Payment is due in full, at the time of your pets discharge from our practice.

We perform dentistry under general anaesthesia, which ensures patient comfort at all times. The anaesthetic will be monitored by our experienced team of vets and nurses throughout your pets procedure and recovery period.
